That NV tool thing is a piece of junk. It doesn't report accurately (at least in my experience) and can have some impossible values, like your 32 on a baseline 100. Try Speedfan.
Pre Fail errors mean that the drive is close to a massive crash. This is usually indicated by a number of things that exceed the normal usage model, like reallocated bits or surface errors. I'd suggest buying a new drive and moving all of your data onto the new one. Quickly.

Whenever I use my PCI card, the driver revs up cpu usage between 80-100%.
