Under the Rebate info area, you can click on the link that says 'Mail the form to me' and they will send you the rebate form via USPS. I've done it before when my printer ran out of ink and they will send it to you.

I was able to get 12 pipes and 5 vertex shaders working.
When I tried 16 pipes and 6 shaders I got tons of artifacts.
I believe most people will be able to unlock at least 4 pipes and 1 shader.
