I put a passive Zalman on mine... or just get another fan close to it... should be runnin all the time I believe....those little fans are pretty crappy... but yours seems to have lasted a while... but if its not locking up..should be fine
I dont want to sound like an idiot.. but could it just be the Inline PowerSupply is bad? if it has one. Might wanna look at that... if you can find an aftermarket one.
Also.. Just a note... from perosnal Exp. Did you try it on another PC? I had a DVI port flake out on me.. once... only...

I often hear people talk about how their Q6600s dont go high enuff.. With air and water I can float around the 3.6 - 4.0Ghz range.. on both mobos Ive OC'd my Q6600 the two boards being DFI LP P35 and also a Biostar Tpower P45.. keep your multiplier and Dividers low.. and see what your FSB...

I really enjoyed 500Mhz X 8 due to higher memory frequency than I did the 440 x 9. the benchmarks said that the higher frequency gave me better results compared to tighter timmings... but each system is different. you try screwing with a 7 multi? and an even higher FSB?
run a benchmark...
why not just oc that chip and see if you like it.... not going to notice a real difference with a quad for gaming...My Q6600 @ 4Ghz during crysis still got beat around at 1920 x 1200 With high settings AA and AF on... dunno if it was the 8800Gtx not handleing it or what... but....your money...
possibly a concave or convex IHS on chip? have you checked the heat spreader on the chip to make sure it is flat and have you checked tightness or snugness of fit while on the PC. Pretty big heatsink. If your using push pins for mounting Id recommend finding a back plate for it. I believe...
