I finally got my 680 back today (9th July). RMA was first issued on 11th June. Took longer than I expected (Scan said their usual turnaround is 5-7 days).
Anyway, they sent me a refurbishment - it's not the same card that I had before (exact same model though). However I'm a little disappointed...

I had to RMA my 680 because one fan stopped working. It's been with MSI for over a week. Personally I wouldn't mind a 770 for a replacement. Anyway I'll let you know what I get back.
Does the setting in BIOS still revert from PCI-E to AUTO?
When you update BIOS you should probably "Load Optimized Defaults" and then go through and check everything.
Is the PCIE slot disabled for some reason? Does the video card work if you put it in a different PCIE slot?

Yeah, the general advice is to never update your BIOS unless it is necessary to fix something - but this is one of those times, it seems.

I've experienced issues like this on Windows 7. I also use Truecrypt containers (not partitions). Sometimes I open a normal folder full of files but Windows Explorer shows it to be empty... usually if I navigate away and come back then the contents are shown properly.

Make sure your Crucial M4 has the latest firmware - a previous firmware had a problem that occurred after 5000 hours use. You said your system was about 6 months old. 6 months = almost 5000 hours.
