Small Optane drive as swap

Yes, they're standard PCIe3 x2 m.2 NVMe SSDs. If you plan on using something that fast as a swap device on linux, a recent kernel (4.11+) could improve performance quite a bit (https://lwn.net/Articles/704478/ goes into the technical details).

Maximus IX Apex. Dual EPS12V. So KBL-S *clearly* is a massive power hog, right? /s. We already know how Ryzen clock/voltage/power scales. Putting two of them in one package won't massively change anything.

You didn't mention what CPU you're using on that RIVB. C0 or C1 stepping Sandy-E by chance? If that's the case, try limiting the slot to Gen2 mode in BIOS.

Minimum card for 4K desktop use?

Rather sad how this segment seemingly got abandoned and/or limited to $$$ "pro" cards. HD7750 Eyefinity cards had 4x or 6x DP, could do 3x 3840x2160@60 DP1.2 SST or MST, required no power connector and weren't unreasonably expensive. 3 years ago.

I7 3960x running at 4.2 instead of 4.6 while gaming

Noticed a similar issue with a E5-1660 on a Sabertooth X79 and Win7. Currently running 47x100, +112mV turbo voltage offset, default LLC, 0 core voltage offset, 1.2V VCCSA and VTTCPU, DDR3-2133 10-12-12 @1.65V. Using single threaded IBT as an example here, but any benchmark, application or game...

Dead Toshiba(Hitachi) DT01ACA300 with under 400 hours in use.

Rectally extracted guess, cold/dry joint on the PCB? I certainly wouldn't trust that drive, but considering it now magically works again with no sign of failure, good luck RMAing it. :|

Samsung 850 Firmware Not offered via ISO?

Pretty sure the only update that appeared for 850 pro was the ill-fated EXM02B6Q. IIRC it was supposed to improve compatibility with some sata2 controllers but in turn caused issues with some sata3 controllers and tended to brick drives when flashed via magician, so it got pulled.

Toshiba 4tb HDD problem?

Got 4 MD04ACA400 and 2 MD04ACA500 in a Norco-ish 4U box here. Attr 191 raw sat at 0 during the initial week of burn-in and in use for a few months. When a 40mm fan in a adjacent switch in the rack went full-on bearing squeal, 191 raw went from 0 to 150-200 on all of them within a few hours...
