The most recent theory is that its the connector not being fully seated into the port on the card. I can confirm that this connector is the hardest PSU cable I have ever seated before. I don't think its a stretch to say that less experienced builders might think the cable is fully plugged in when in reality it isn't. I had to hold the back of my case with one hand and push with nearly all my strength to get the audible click when inserting the plug. The plug heats up to insane temps quickly when not fully seated according to some recent reports. I am using a cablemod 4x8 pin to 12vhpwr cable. Never tried the Nvidia adapter. Checked my cable a couple times for melting and so far so good.
