A collection of papers giving detailed information about a particular person or subject.
1) Warning members of a violation ? For isolated incidents, first time offences, or minor infractions, moderators will issue a warning to a user in the form of a Private Message (PM). Along with this, a note may be made in the users profile about the incident and warning issued. Warnings will be given at moderator discretion based on member behavior or the need to moderate a member's threads.
2) Temporarily banning a member (vacation) ? If a member receives multiple warnings, or commits a serious violation of our guidelines, the member will be temporarily barred from posting on the forum. A note will be made in the users profile about reasons and length of vacations. Initially vacations will be short (a week), but if a user has received multiple vacations, moderators can grant longer vacations (multiple weeks or months in extreme cases).

I think the confusion is on what exactly constitutes a "dossier".
So, pretty much any notes on members could be considered a dossier. It would be insane for the mods NOT to keep records. How would they keep track of the "trouble makers" if they didn't? In fact, the Forum Guidelines state that there are notes kept on users:
I have no idea what goes on behind the scenes with the mods, but couldn't a "note may/will be made in the user's profile" be considered "collection of papers [electronic, in this case] giving detailed information about a particular person or subject"?
There have to be records kept, so couldn't those be considered a dossier?

Everyone needs to just take a deep breath, log off Anandtech, log off Facebook, log off Twitter, and then SHUT DOWN your computers, iPads, tablets, smartphones, etc. DISCONNECT. Go outside. Breathe in the outside air. Call a friend or loved one. And engage in direct, human social contact for at least the next 12 hours!

Ah, the Change of Culture. I remember it fondly. Like a colonoscopy. These forums are a private club. When those in power decide you are no longer cool you're gone. That "change of culture" was basically the mods deciding what was good, what was bad and what we like today at this moment. Rabid trolls that contribute nothing were allowed to roam freely b/c they new a mod, while intelligent members who actually had something useful to contribute got swatted. There was nothing fair or rational about the process. If you were dumb enough to stand up for a buddy, you got swatted too.
Most of the members that got swatted are back; half with a new username.
Very few people are permabanned. No. They just rot in in "we're still thinking about the length of your ban" purgatory forever, never to be heard from again. Conveniently.
I was one of those people that got swatted during the Change of Culture. I'd still be banned if not for one Mod who shall remain nameless who thought about me more than a year later, looked at my status and emailed me to say "Um...you want back in?"
