Any lantus users here?

Don't know what that is, but Lantus is a long acting insulin. My daughter takes it before bed to regulate sugar while she sleeps.

levemir is also a long acting insulin. i take it along with novolog, which is short acting.

I was on it...didnt care for it and the many shots a day, so I got a pump instead.

Yes. works for me. No insurance so can't afford pump. Maybe someday. My favorite "side effect " is- may cause low blood sugar. Huh? That's not a side effect, that's the whole point of the drug.

1. How much do you guys take?
2. Does it seem like it starts to loose effectiveness, and you need to "up" your dose?
3. Do you suffer from "dawn syndrome"

You're type2? That 's probably different from my daughter's experience. I think type2 will be more variable, and you can need increased dosages, but don't take that as gospel. I'm not intimately familiar. If you're heavy, REALLY try to lose some weight. That should make it more manageable. Limit carbs also.

My daughter@120# takes 24 units before bed.

I don't know what dawn syndrome is.

1. When I took it, I took a total of 44u/day split into two doses. One at night, one in the morning. Daily basal requirements are PURELY INDIVIDUAL. It speaks nothing of control or anything else. Ive met obese people with type 1 who take less than 30u/day, and Ive met triathlon athletes who take more than 50.

2. Metabolism changes, so for some its a possibility. Also, insulin resistance can change over time as well.

3. If you mean rising blood sugars in the early AM, then yes. Its pretty common. Im on a pump now, and my basal rate increases about 35% between 3am-11am. Its very common.

I had originally started taking about 28 units per day, but over time have had to increase my insulin to get the same numbers on my routine blood tests. My weight is unchanged, and my workout routine is the same (3-4 times per week, 45 mins bicycling). It seems to me that my body is building a resistance to the Lantus, but I am uncertain.

Just a thought since youre type 2...you could be experiencing whats called β-cell function failure. HERE is a clinical paper about it...dry reading I know. Also, if youre seeing an endo, it may be time to start doing regular c peptide tests to measure pancreatic output. With type 2, it can change. If youre not seeing an endo, you should

I always love me some diabetes threads....

Type 1 for 20 years. Was on NPH with regular, then NPH with humalog, then ultralente with NPH with humalog, then ultralente with humalog, then humalog insulin pump, now novolog insulin pump but when I am not on that I use lantus with novolog. (thats a mouthful....). The point where I had NPH with ultralente was trying to deal with dawn phenomenon but that was many years ago. My morning basal rates (1 to 6 AM) I use now are actually lower than the rest of the day these days. (1 unit vs 1.3/1.4/hr)

so with that out of the way....

1. On the pump, as above, plus 1 unit/10 grams carbs and 1 unit per 50 over 150. On lantus, I take about 33 in a single shot each 24 hours, and the correctional stuff stays the same. (I weight about 150 if you care although as said above that isnt really necessarily a determining factor, however I have noticed that as my weight increased I needed more basal insulin. YMMV....)

2. Actually, right now I am getting the opposite effect -- I was becoming, shall we say, a bit.....sedentary.... and had to take more, now I am a bit more in shape (exercising every day) and using less. Lifestyle has a massive effect for me.

3. Not any more. A pump is great for that though. And great for pretty much everything, in fact.....assuming you have insurance to cover it.

edit: Something else with diabetes to note: there are a number of ways that your body can becoming more 'fail-y' over time. For instance, you can develop resistance or b-cell failure......in my life, I notice that over time my body's ability to 'buffer' against a low blood sugar has decreased over time (ie, a low used to be 65, now it is 40, etc.....) which is related to hypoglycemic unawareness (potentially very dangerous) as well as diet and stores of sugar in the body but also might have to do with a decreasing glucagon activity in the body (although I'm not completely sure on that). Anyway, good conversation to have with an endocrinologist.
