Best contact form generator

I'm looking for a good contact form generator that can do checkboxes, radio buttons, text fields, etc. and includes the script file (a lot of nice ones use scripts that are hosted on the company's site... i want the script file on my site).

Any suggestions?

formstogo is awesome and does everything you request. Will also use a Mysql database if you want.

I personally own the PHP version. Takes a little clicking around, but its pretty self explanatory.
