For those of you who still need the reabte form from that CompUSA/Kodak 50 pack CDR deal I have it for you!! I emailed Kodak and asked the rep if I can have a copy. He emailed me the form and I am making it availiable to all.

If you are wondering if you can send in a printed out copy I would say yes. This is the email from the rep:

I am an idiot! I was in a hurry to open the cheap plastic packaging that the Kodak CDRs came in, and I ended up chucking the label along with those "wanna be" spindles. Anyone know if its possible to do the rebate w/o the UPC? I have the receipt.

Just wanted everyone to know that the Adobe pdf rebate form I posted is valid. I was concerned because in the fine print it said that copies would not be accepted.

I went to www.rebateshq.com and called the 800 phone number on their website. Interesting thing is at the main menu it said to push "1" for the Kodak rebate. Interesting...sounds like they have been getting a lot of calls about this. Anyway I pushed that and I talked to a rep. She told me that a printed out copy of the pdf file is accepted.

Just wanted to post this in case anyone had any doubts.

After reading the fine print on the bad photocopy of the rebate form which CompUSA had given me, I took it back to the Culver City, CA store from where I had purchased the disks, and to my surprise, the customer service guy did the rebate on the spot, giving me back $20 plus the tax!

Of course, I had to drive there and wait in line for 15 minutes before this happened. But, with both cash and disks in hand, I can't really complain.

Keyzer - I did the same thing by accident...did you figure anything out?

All I have is the receipt, rebate, and the two hard plastic outside covers, and one of them has about half of the original Compusa number on it ( 068 4), but you can tell its a compusa upc. I was just going to send all of this in - will this work?

I'm bringing this one to the top because the saga continues...

Yesterday, I received a post card in the mail indicating the $19.99 rebate would not be paid out to me from CompUSA on these CDRs because mine was postmarked after 9/26/00.

It turns out that my local CompUSA (Fair Lakes / Fairfax, VA) didn't have the rebate forms in stock. CompUSA (referred to as ChumpUSA from here on out) mailed me the coupon a few days before the rebate was to have been sent in. I still sent it in and held on ... until I received the rejection notice. Being an anal person (no, not the way y'all are thinking, but rather one who holds onto documentation), I kept the envelope from ChumpUSA in the event I had to defend the rebate.

Today, I brought all of my documentation into ChumpUSA, and after their review, I received my $19.99 rebate in cold, hard cash. Also, the customer service person indicated that she had seen several of these come in due to rejections of the same nature.

The moral of my story is that if you're struggling with a rebate, then head to the store of purchase and usually things will work out to your advantage.
