Its really simple. All you need to do is the following:
For the droid serial number:
Take the number of your current address and add a colon
Then add on your social security number backwards (remember those dashes) and a space
Tack on your mother's maiden name and add a space
Now put down your zip code plus the extended 4 digit number
For the model number:
Put down your current street and then add a dash
Put down your last name and then add a space
Use your last year's income and then add a dash
Finally put down your phone number backwards (no dashes)
To be really cool, come up with a special designation (You want to be cool right?)
Put down the name of your current employer and then a space
You're boss's phone number and then a dash
Finally put down the number of years you have worked there.
