Before I get started, I haven't actually received my rebates yet as they are currently being processed. By all indications however, this should work. (Steps are below if you want to skip the details)
Requirements before you get started:
1. Must be a Costco member
2. Must have a receipt for a contact lens fitting and exam (either through Costco or a different optician)
PURCHASE MUST BE MADE BY JUNE 30th, 2006
Costco is currently offering a $50 rebate on Acuvue Contacts when purchasing four or more boxes of any of their "Ultra Comfort Series Lenses". Acuvue is ALSO offering the same exact rebate. The difference is that Costco does not require the box tops and you can fill out a rebate form online. All you need is your Costco receipt info (a couple numbers off the receipt) and that's about it. Acuvue does require the box tops, so essentially you are doubling up on the rebate offered.
Steps to take (preferably in this order):
Step#1
Go to your eye doctor (I used Costco which charges a little under $100 for the exam, fitting, and free trial pair of lenses-no insurance used). Make sure that you tell them you want Acuvue Contacts before you get started. Get your contact lens exam and fitting receipt from your doctor before you leave.
Once done with your exam, request a rebate form for the Acuvue Rebate. If your doctor doesn't have one, you can get one HERE
Step #2
Order your Acuvue contacts through Costco (assuming you have an optical department-go there). If they don't have them in stock, it generally takes about 7-10 days to get them in.
At that point, the salesperson will charge you for the contacts and give you a receipt plus a yellow receipt form that you can fill out for the rebate. I WOULD SUGGEST DOING IT ONLINE THROUGH COSTCO's REBATE CENTER THOUGH. It's much quicker. You can find the Costco Rebate Center HERE
Step #3
After obtaining the boxes of contacts from Costco, use the Acuvue Rebate Form. Cut off the UPC box codes on at least 3 of the 4 boxes Mail that in with the copy of your Contact Lens Fitting Receipt AND your Costco Receipt for the Lenses (Not the yellow rebate form) to the address on the Acuvue Rebate Form.
Note that everyone's deal will be slightly different as Costco's prices for each type of Acuvue Lens varies from about $14.99-$29.99 a box.
Lastly, here's the math for my particular lens I need on my left eye which has astigmatism:
$29.99/box of Acuvue Advance for Astigmatism
4 boxes = approx $120
Subtract $50 through Costco
Subtract $50 through Acuvue
$120-100= $20 for 4 boxes of contacts.
