According to this site call 702-269-3457 Press 2000. I don't know how recent this is. Also here has a different number and instructions.

Same thing happened to me. I called, they said "did not receive a rebate certificate" and placed me on hold to verify the info. She came back and said "your rebate will be mailed in 4-6 weeks."
