I can't say how Electroconvulsive Therapy (ECT) is administered in Croatia, or where ever, but in the US it is administered under general anesthesia. The patient is not aware of it.

ECT has proven effects in patients suffering from severe clinical depressive disorders that are unresolved by other first-line treatment options. The improvement is often quite remarkable and the therapeutic efficacy is often superior to those yielded by medication and psychotherapy.

It is also used for extreme cases of ADD. A cousin's son went through that. It's been 2 years now, and according to my cousin, it's worked wonderfully.
