Yes, I've done that and the saying hello to someone I thought I knew but don't. That one is more awkward.

That has never happened to me before, not to my knowledge. I'm cautious when it comes to introducing myself, but rarely they call me by the wrong name.

Never forgotten my own name, but I did introduce my girlfriend at the time with an ex's name to my family at Thanksgiving one year. That went over REAL well.
