Finally got my signature card. Did anyone actually read it?

b) I am not subject to backup witholdings because, 1) I have not been notified by the IRS that I need to have back up with holdings, 2) the IRS blah, blah, 3) blah, blah. [Cross off item b if it does not apply to you]
c) I am a US person
d) I am not a US person and I have attached form XYZ.

Let's see....b is the dreaded tripple-negative, and c & d can't both be true.
I called E*Trade to ask them about it and even the CSR said it was poorly written.
Seriously, what's the right thing to do for "b"?
