Ok. Here's how to get it to work every time.

If you click on any of the links in this thread, and it says your session timed out. Scroll down to 'Contact Us' and click on it.

The first thing you will see is the option to get a free Pizza and coke without taking the survey. I did it, and it worked fine. You will get a Visitor Code to send along with a SASE.

Still it requires you to send an addressed envelope...

Whatever you do, don't fill out the form online, it authorizes them to bill you for advertising. If you put in another phone number, that constitutes fraud.

That Madison Ave address (google search on the address) is listed under a lot of different places so is probably a rentable address for companies to look like they've got some big office/presence in Manhattan.
