When the medical consensus is that vaccination and masks save lives, I believe that bad actors without countervailing reasons not to comply should lose their rights to intermingle with the public generally.

Is it genuinely fascist to give people a choice where the refusal is a consequence of their own actions? Is it even authoritarian to give people that choice? I don't think so. Specifically when they can get tested weekly instead but refuse that as well.

I don't think it is, if it is then requiring you to wear clothes when intermingling is clearly fascist as well, is it not? Or is social standards somehow higher than medical advice in your mind?
