I already offered a solution, that his premium costs can be capped like those of people who earn less if we're willing to quit white knighting for the Plutocracy. It's not all that many people so we won't have to raise taxes at the top much at all.
I think you're going down the right road. If a cap is going to be in place, it can't stop at 4x poverty. However there'd have to be something that keeps ins companies from charging as much as they want under such a system too.
