Given the medical background/history; I would expect that she is a O4 - Lt Commander (Gold Oak Leaf)

Medical degree almost always gets a noob in as an O3 - Capt(USA, USAF, USMC) or Lt (Navy/CG). (Insignia is Dual bars)
