The card they tested was already factory overclocked and they were still able to push it a little further.
Using 2 of the latest Xbitlabs reviews the OC 4850 1GB is faster than OC GTS250 1GB. They didn't directly compare the 2 so I had to make my own chart using the info from the 2 reviews.

Then it's hardly worth mentioning.
Both review use the same test bed, driver config, and are from the same site. Besides the GTS250 got slightly more updated drivers. so it's worth mentioning.

That depends on the game library.
Not only that, but the performance hit is a bit of a misnomer these days; 8xS actually has a smaller performance hit than 16xQ and 24xAA, and probably 12xAA too.

Good point. I noticed the same thing, at least when comparing it to 16xQ.

Question:

Are all 9800gtx's 65nm? And are all 9800gtx+'s 55nm? Or are some 9800gtx's also 55nm?

i think only the the 1gb versions of 9800gtx+ are 55nm.
also, i think some gts 250 512mb versions, might still be 65nm parts.

9800 GTX = 65nm
9800 GTX+ = 55nm
GTS 250 = 55nm

There was no overlap like with the 9800 GT. From day one, the GTX+ has been 55nm.

With that being said, I've read forum reports stating that some have gotten 65nm chips on a GTX+. Turns out that some manufacturers just re-stickered a GTX as a GTX+. This is not the same as an NVIDIA-authorized rebranding. It worked with the original GTX+ as those reference boards and fansinks were identical to the GTX.
