But, I found someone ranked 439th - his mhz rank is 1094th, and his kbps rank is 1109th. I'm ranked 432, my mhz rank is 354th and my kbps is 321st. Clearly, bandwidth and megahertz have very little to do with the ranking. I don't think it's credits either - I've been breached once, for a grand total of 35 credits - I keep them spent or in my safe.

Upgrading your virus and vault also improve your rank. Also upgrading the virus is the best strategy in the long run. The game is fun, but it eventually it gets impossible to usurp the top few players.

Currently ranked in the top 20

When somebody opens the following link and completes the captcha, you will gain whatever your "Current infections per tick" is. Each person may only click on this link once every 24 hours and there is a maximum of 5 per day.

Apparently, only Leros can do the inviting. I don't think that just clicking the referral link works - the system also keeps track of who a referral was sent to. If you're not invited, then figuring out the correct link doesn't do it.
