2. Same as 1, but now you also cannot swallow and need a feeding tube.
3. Same as 2, but now you also cannot breath your own and need 24 hour ventilation.
4. Same as 3, but now you also cannot speak any more.
5. Same as 4, but now you are also deaf.
6. Same as 5, but now you also blind.
7. Now you are in a vegetative state.

I would say #1 assuming technology and medicine remained the same as it is today. If given the hope that research could one day solve my issues, I don't know what I would choose. I don't know enough on the subject to make a decision.

That's a tough question AND not one any of us want to face. When I thought of being entirely dependent on care givers... permanently... that's not pleasant. I had to side with option 1 though I cannot truly say what state of mind I'd be in at the time.
