In my life I have seen three great danes of abnormal height(for the breed), and they have all looked that way. The dog in the picture is over a FOOT taller than a normal great dane. I'm sure the dog has a vet and the vet thinks its fine.

The one on the left has zero body fat and has visible vertebrae and looks frail and hungry. The woman on the right has a generous but not out of control amount of body fat.

For any other breed, I would say underfed. But Great Danes can be very boney and they fill out very very late. If that dog is less than three or four years old and the vet says he's OK, then he's OK.

The ribs SHOULD show a little in a properly-fed dog that doesn't have a thick coat. If you can see the whole ribcage, or if it has a thick coat and you can STILL see the ribs, you might be in trouble. The one in the OP looks a little underfed to me because of the prominent vertebrae, but it seems that this particular specimen is taller than usual. Because of this, it makes sense that it might look lanky and awkward despite being well-fed, just like too-tall people can look malnourished when they're not.

Underfed but I have seen far, far worse. Ribs are one thing but the lack of fat with spine and the hips showing are a dead giveaway.
