As for the size of the explosion, two things come to mind. First, hospitals have a lot of oxygen canned up. Two, Hamas has used various buildings to store munitions before.

Russia - Embarrasses the US President right before his arrival and keeps the mess going in the Middle East while they pull f$#kery in Ukraine. This also increases the probability of getting their friend back in the WH.
Iran - Keeps Israeli from normalizing relations with Saudi Arabia. Which keeps Sunni's from aligning with Israeli and presented more of a threat to Iran.
