We had a program for making these on a daisy wheel printer. It would use over-typing (backspace and over type with another letter) for areas needing more ink than a standard character could provide. When I was a little kid my uncle who was an engineer @ Hubbell had a very early dot-matrix printer and used to have these hanging in his office. (think well before Okidata)
Although obviously not of the Princess-bride... I recall Mona Lisa and other "classic" art type stuff plus the occasional comic/cartoon character and even a few family-photos.
Same guy had a super-early microwave oven that was the size of a medium fridge on the outside but had a cooking volume of roughly 1 cubic foot!
