I finally got through to contact at
Free800Number.com. Appearantly he's an independent agent for Global Fibernet. I asked him why I saw rates ranging from 3.5 cents to 3.9 cents/min on the same Global Fibernet site and he informed me that each agent sets their own price but generally uses the same page template, so make sure you shop around.
I signed up for the 3.5cents/min. plan to call anywhere in US which is the lowest DIRECT dial I've found (thanks posters for confirming!). He pointed out FAQ on
website that there would NOT be any monthly fee, no prepays, no access numbers to dial and all calls would be rounded to 6 sec. increments (my current carrier is 60 sec than 15 sec after that) and that if I do not make any calls, there would not be any charge.
Overall this is the best direct dial calling plan I've found. Only thing is that
Global Fibernet only sells through independent agents so initially trying to speak to a salesrep live may take a little longer. I did get a toll-free "customer service number" for their corporate office after signing up, which he informed me is what I use once I'm a client. I tried calling and got a live person during normal business hours first time, so this doesn't appear to be a problem.

PS - I asked if I could get a personal 800 number like 888-123-COOK that would ring my home office line, he said if the number is available, there's no cost to request persoanlized toll free number, and no monthly fee for the 800 only I pay for each call at 3.5cents/min.
