Maltron vs Kinesis vs Generic TKL

I'm planning to go into programming as a career, but I already have carpal tunnel and etc.
I also plan to switch to a quasi-DVORAK layout.

But I'm curious about the designs in my title.

I'm drawn to Maltron because supposedly its ergonomics are slightly better than Kinesis, and because of all the extra keys it has in the center.
I'm drawn to Kinesis because it supports macros and offers cherry red (which is what I use now).
I'm drawn to generic TKL designs because they allow the mouse to be placed closer to you. The malton and kinesis are so wide that using your mouse requires your arm to be angled outward.

So, thoughts? Which should I get, and why?

If you already have carpel tunnel, I'd focus on solving that issue asap.

Find out what your hands like and dislike--this is the first step. Then explore what your hands do like, searching for what allows them to type pain free endlessly. I was lucky enough that my choice was easy--the venerable Model M. Good hunting!
