This isn't an american phenomena. I've saved a few folks lives thanks to my training and in EVERY situation people just stand around, unable to act. There's an actual psychological term for it but I can't think of the name. People really are like sheep in an emergency, they will just stand there and look. That's why number one thing you do first is bark orders at them. And when i mean bark, I mean look them in the eye, take control of them and TELL them what to do. They will follow. If they don't, then move to the next do the same.

YOU! Go grab a blanket
YOU! Hold his legs up
YOU! Support his head and watch his eyes, yell at me if he stops breathing.
