You're talking elementary here, Yes the fist is hard but the knuckles are only covered/protected by a very thin layer of flesh.
Dude, that is one of the most calloused spots on your body. That part of the hand, along with finger tips and parts of your are parts of the body designed not to rip open or tear nearly like the rest of your skin. Of which, skin isn't really all that hard to tear or open by design. Yes a sharp object can rip open skin, and soft skin can be more easily braised than calloused skin, but it still takes a lot more force than what is going to be reflected back by the mostly smooth surface of the average human face.
