when i was young, Top Gun was a pain in my ass when trying to land on the carrier. i think i only landed on it one time.
there was this one level in Max Payne in which hes drugged and is in a dream sequence. All you could hear was his child crying and complete darkness everywhere except for a thin pathway.....God, that was haunting and bloody frustrating cos i kept falling off...
My all-time gaming nemesis is Kid Icarus on the original Nintendo.. I loved and hated that game all at the same time.

I think those old school NES games really take the cake in difficulty. They were generally quite short, so extended playtime came about through frustrating difficulty--sometimes achieved through poorly responsive controls.
But the games were awesome. Another frustrating game, but also quite fun: Faxanadu.
Ninja Gaiden 1 & 2--the real Ninja Gaiden, not the contemporary versions.

Ninja Gaiden 3 is without a doubt the hardest of them. Both 1 and 2 had unlimited continues, 3 did not.
But don't forget Battletoads. If you ever made it past the turbo tunnel, you still had to deal with the snakes level (don't remember the exact name), where just about every other jump you had to make was a blind leap to a spot that the snake will be in the next few seconds (but which you do not know until you have seen it and memorized the spot from dying there a few dozen times, only to have to play the entire game over again to get back after your 3 lives are up).
Battletoads was beatable for one reason. The decending through a tree on a string level could give you as many lives as you wanted by beating the same bird against a wall (for progressively more points and eventually a 1-up per hit). I actually legitimately (that is without a game genie or the like) beat battletoads with this.

Tried QWOP and managed to beat it by using only the Q and W key. The hardest part was going over the hurdle in the 50m mark
