There are different meanings for "screwed up".

Requiem for a Dream
(screwed up = a dark, sad, tragic plot)
Note: this is the only movie I've seen so far that is powerful enough to make me feel physically ill.

(screwed up = difficult to follow WTF happened)

If you mean most tragic and just the greatest amount of human suffering ... "Requiem For a Dream" and I think also "Ran" would rank very high.
If you mean violently offensive and hard to watch .... flower of flesh and blood is horrible (as well as the other 2 movies in the series)
If you mean just straight disturbing ... Julien Donkey Boy, Eraserhead, Salo, and Gozu all have their moments.

Others would be Bully, Vulgar and Funny Games (that movie fucking pisses me off).

I have yet to see Eraserhead, Gummo, Straw Dogs and A Clockwork Orange.

have you seen Sympathy for Mr. Vengeance? that left me even more...WTF. I actually enjoyed Oldboy and was able to get into the mood/story, whatever; so it became less shocking by the end.
