I cringed my way through the movie. Truly uncomfortable, wincing at the screen, wondering how much fucking worse things can get (onscreen, in the storyline). I didn't like the movie at all but I get its purpose/theme. Some truly fucked up stuff.
'I Spit On Your Grave' didn't shock me. Just a real boring, amateurish movie that looked like it was shot for $5000. Yeah, she screams a lot in the rape scenes and the depiction is revolting but it felt more like 'realistic porn' designed to titillate rather than 'OMG! Ultrashock, look away from the screen!" (The Bellucci rape scene in Irreversible -> OMG, look away from the screen)
Aronofsky's movies are haunting/trippy and do feature screwed up people/storylines but not exactly what I'd call screwed up movies.
A Clockwork Orange is a good pick. That Alex DeLarge kid was a goddamn monster.
