Tokyo Gore Police doesn't really qualify but you'd probably like it anyway. Follow it up with the two Hard Revenge Milly movies.
A Uwe Boll film with a ~6.5 rating on IMDB
I watched it. It wasn't very good, but it was far more watchable than his others.
Anyways, if you truly want to watch something disturbing, there's plenty of stuff to choose from. Just go to television shows, then select the reality TV category. There, you can watch the craziest, most assinine, idiotic, and repugnant behavior known to man. Afterwards, you realize that the majority of society acts in such a manner. If you aren't disturbed after that, then you are unable to be disturbed.

A Serbian Film if it's available
I gotta see that.
Does NetFlix have the uncut version of Bad Lieutenant? that was pretty disturbing.
Rampage... flick about a guy in an armored suit murdering a town.
I have to say, out of all the ridiculous "reality" TV crap that I've seen other people glued to (Survivor, Big Brother, etc.), I watched Out of the Wild: The Alaska Experiment on a whim. It was really good.
Anyway, back to your regularly scheduled creepiness discussion.
Not a movie but Elfen Lied quite disturbing. All 13 episodes. the first 3 episodes are out of order, watch 2 -3 -1
9 Songs is on Netflix, and it's not a porn, but shows a couple actually having sex throughout the whole movie. Pretty shocking
