Nexus 6 won't autofocus?

Wife and I both have Nexus 6s with Google Fi and love them, but a few weeks ago her camera stopped autofocusing for some reason. We're both on the most recent official version of Android 7 for the N6.

Her phone hasn't taken any hard hits and we both have thick, rugged cases that should protect them well from normal wear and tear.

My best guess is that there's a setting somewhere that she flipped accidentally, but haven't been able to find anything about it on Google. Any ideas?

As simple as it sounds..is the lens (or laser focus if it has one) dirty?

Figured it out. Glass plate over the lens came out. Derp.
