Act 1
You start in the Tamoe Highlands (I think that's correct). Some crazy bitch is a demon and started killing some travelers, rogues came to fight them, but got corrupted in the process. You visit Tristram, save Deckard Cain, kill the main demon bitch.
Act 2
You're taken to some desert city, the mayor guy tells you stuff is happening, help. You go through and get stuff to open Tal Rasha's tomb (Mephisto was trapped by some guy, Diablo is breaking him out). You kill Duriel and the archangel Tyrael tells you about Diablo, says to stop him.
Act 3
You're shipped to a jungle, they're having problems, their magic shield is weakening. You help them, find out about some high council helping Diablo and Mephisto do shit. Kill the high council, break the orb, go into the temple where Diablo and Mephisto are trying to get back to hell. You get there just as Diablo goes through, kill Mephisto and then follow through to the planes of Hell.
Act 4
You're in hell. Tyrael tells you to kill Diablo. You break the shard holding Mephisto's soul on a forge. Make your way to Diablo and kill him. Tyrael says good job, but Diablo got Baal back and he's fucking shit up on Mt. Arreat where the Worldstone is. That's bad.
Act 5
You're in a barbarian city under siege by Baal's army (which is just a diversion to stop people from stopping him getting to the Worldstone). You follow Baal, get tested by the some ancient barbarians called the Ancients, they let you into Arreat Summit. You find Baal, kill him. Tyrael shows up says the Worldstone is corrupt, the world is basically screwed now. He destroys the Worldstone. The end.
If I have it right, Mephisto (the runt of the litter) is dead for good. Diablo and Baal are dead in the mortal realm, but their souls still remain so they can come back. There's some speculation as to what happened at the end of Diablo 2. Some say that Tyrael was corrupted, possibly possessed by Baal. Essentially, the gateway between Hell and earth is open, which will allow their armies to enter the mortal plane, whereby they will then wage war against Heaven or something.
