Tony wakes up in a hotel room and looks confused. He sort of grabs his stomach like he has indigestion. You see him in a bar then, and he's having a drink. He gets up and calls home, getting an answering machine with an outgoing messege that sounds like two younger kids, a boy and a girl. He tells them he got there OK and that they should call him back. He steps back to the bar, gets his wallet and briefcase, then walks outside, again feeling his stomach like he doesn't feel well. He hears a helicopter and looks up into the searchlight, briefly you see someone flashing a light into his eyes.
Next day, he goes to a conference, looks like a big military vendor meeting or something. He tries to register, but doesn't have his own wallet. He goes back to the hotel, but they won't let him in his room to look for his wallet. He goes back to the bar and meets some people and has dinner and drinks with them. He calls home again and finds out the guy who he accidently switched wallets with hasn't called back. He returns to the table to find out that one of the guys paid for dinner and left. Next scene is Tony outside of the hotel making out with a woman from the group of people he had dinner with, but she says "it's not going to happen". They both look up into the helicopter searchlight again, and this time it fades into a three-light OR-style light fixture. Tony is in reality, in a hospital bed hooked up to a breathing machine and is in bad shape. He pulls out his breathing tube and struggles until they sedate him and put his tube back. Carmella and Meadow are there in the room and the doctor makes them leave.
We find out Tony is in a medically induced coma and that Uncle Junior Soprano is being questioned for shooting Tony. There is a funeral for Gene, and Silvio decides that he will collect for Tony as usual, and that he'll give some money to Carmella to run the house. Vito wants the sportsbook that Gene ran, but Silvo tells him it isn't time to be asking for something like that.
Tony wakes up (in his coma) and walks out of his hotel room. The elevator is out of order, so he takes the stairs, where he falls and hits his head. That's how he ended up in the hospital (in his coma).
The doctor tells Carmella they should play Tony's favorite music and talk to him while he's out. Carmella has a talk with Tony saying she feels terrible about saying that Tony was going to hell and that he shouldn't go to hell. If Edie Falco doesn't win an emmy for this scene alone, there's something wrong. She's also upset because AJ won't stay with Tony and watch him. AJ ends up deciding to stay the night with Tony and talks to him about cars. In the morning, AJ tells Carmella that he flunked out of college.
I might have gone into what you already saw - but that's most of the episode in pretty good detail. I probably left out some of the non-coma stuff.
