Actually, geek0rama's post alerted me to the fact that OMAX has the incorrect rebate form linked on-line at the Kingston Ram OMAX product page. They're referencing the old JUNE (2003_06) PDF file while my OP is referencing the current July (2003_07) PDF file. Although the Cendyne's a slightly slower drive (on the RE-WRITE speed), the I/O Magic drive (which has the incorrect MFR REBATE FORM linked at the moment) will likely involve a true I/O MAGIC MFR REBATE FORM. I/O Magic / Hi-Val rebates are IMO the absolute worst rebates to deal with, PERIOD! They take forever, and a majority of the time you have to RESUBMIT your rebate info to get paid. I personally will never buy another I/O Magic / Hi-Val product involving a MFR REBATE.

However, the July PCash1.pdf file has an asterick ( * ) only next to items that can have a copy of the UPC sent. Called OM today and they're requiring the original UPC on the Western Digital 80GB drive for the $10 rebate. So how can I send for the OM $10 rebate and the $30 WD rebate?
Do I send the capacity label to one (as in the picture) and the barcode to the other?
They're both going to RebatesHQ but to different mailboxes...
