Nice post, I don't mind reading long posts at all actually. Curious to know what's your take on universal health care? Works well enough for most other countries. At least from a patient perspective the system is a LOT less stressful and the waiting times are not what the rumours make them out to be though they are sometimes an issue. Only five patients aquired after two years; could you not see the trend sooner? If determined to remain there then it seems the only option is to look beyond the emergency room to find more. Have you talked to colleauges or mentioned it to the existing patients?

How does your billing work? Have you tried asking for payment in installments rather than lump sum? It must be a significant sum owed by those 475 and so the potential for recovering even a fraction must be worth more effort.

As alluded, perhaps those people are not even aware that you personally are getting the shaft as opposed to the hospital or "the system". In addition to installments is it feasible to offer the deadbeats reduced rates for ongoing services? One would imagine that some would be glad to have a regular doctor if only they could afford it.
