I just don't understand why going with dell/lenevo would be any better, especially if I were to choose good quality hardware.

The difference is that in a year's time, you will still be able to get parts for the Dell/Lenovo, but will have to swap in a different motherboard/RAM/PSU, etc. into your self-built machine (at your expense of course). Now you have a non-homogenous environment where different machines have different quirks (especially in the case of mobo replacement).

oscar6 you should take into considerations that hardware is the least important factor in Computer system for medical Practice.

The real issue is how to configure everything to be HIPPA Compliant ( http://en.wikipedia.org/wiki/Health_Insurance_Portability_and_Accountability_Act )

If you do not know howto you should first take a consultant to help you in the decision making.

Support I've never used, but with the ATX mid-towers, the mobo and ODD are all that most people can't get locally, that might fail unexpectedly (RAM, HDD, PSU, video cards, and network cards can usually be had locally, somewhere, even if you're not in a metro area). With that, there's no negative, compared to white box, at least when buying the business lines.

I've used the support a lot of times, but pretty much for parts. Next-day parts without have to pay is pretty nice. You can of course scrounge locally if you need same-day.
