There is a bus directly to JFK from Penn Station - I would recommend that as it is easiest.
Otherwise take the E Train to Jamaica and then the Airtrain

It stops at all terminals; you still have to walk to your gate.

i know it's closer but it's not on a rail line. so you're limited to shuttles and cabs, aren't you?

A cab from midtown (say Penn or Times Sq) to LGA will cost you about $25-27 and about 30-40 mins depending upon the time of day, traffic and whether you're taking the Queensboro Bridge or the Queens Midtown Tunnel.
If you're traveling at 6:00am first flight out, then the cab ride will cost you closer to $21-22 and you'll be there in 20 minutes or less.
If you have more time than money, take the N train to Astoria Bl. and switch to the M60 bus (which is very regular & frequent) and will drop you off in front of your terminal.
Getting back & forth to JFK is a freakin pain

Huh? Getting to JFK is far easier with subway (A/C) takes you straight there and then that automatic train. With LGA you gotta go to Astoria and then wait for a bus...
