Putting forms in a Picture

How would I go about doing this? Anyone know a tutorial or can post on how to do this. I have access to CGI to send the results but to make it clear for my viewers I would like to have a picture so they can Fill it in as they go along. Thanks.

that picture doesnt make much sense from what you described. what are you trying to do? i see a bordered image around the edge of the form. if you wanna do that on the web, you either use flash or leave the form in a table cell.

no the white boxes are where i want the forms to be. Thats the screenshot id like to use. Is there any possible way to do that?

I'd guess you make a table with the image background being that image (without the white spots leave it black) then create cells and try to make the input boxes in the right spots. Or you could do it in flash, but I don't know flash at all so I can't help you out Good luck.

heheh thats exactly what i did See It Here.

I used a layer so it would keep its original size and used a table. The spacing is a little off but hey, least it works
