1. EAC with AccurateRip.
2. Convert to lossless with WavPack.
3. Set tags from filename (and clean up tags) with Tag&Rename
4. MP3 encode using Foobar2000 (which will automatically copy tags from the WavPack files to the MP3 files as it goes). Foobar2000 uses Lame (or the encoder of your choice) behind the scenes.
AccurateRip checks ripping results against a database and lets you know if your RIP was perfect or not. Lets me rip in burst mode (which is usually perfect, but occasionally not) which is 3-4x faster than the normal ripping speed. If I get a bad result on a track, I just rerip that one track (once every 20 CDs or so). YMMV depending on your drive.
