After reading this thread, I decided to play System Shock 2. I'm not sure how far into it I am but it has definitely had some moments that made me jump. The feeling of being completely alone on this ship, with only the random hybrids mostly all you see, it lulls you into a sense of calmness, then BAM!! Hybrid appears as you turn the corner. That along with the voices and cyborg midwives is enough to propel it to near the top of scariest games.
Back in the day, when I was about 12 or so Wolfenstein and Doom with the demonic references freeked me out.
But Silent Hill 1 and 2 are probably the scariest games I've played.
BTW, if you haven't played SS2 you neeed to. It is an extremely fun game.

it is also the first real "hybred" game with RPG elements melded very well to a FPS shooter ... it seems to have 'inspired' DeuSex [i'd say they and most other popular games that followed ripped ideas from it]
unfortunately when you play it NOW, you lose the sense that this was the "first" game to implement so many innovate ideas back then
{it had some *crap* jumping puzzles, if i remember right also}
HOWEVER, i never had the sense that it was "scary" ... atmosphere of horror with suspense [and irritation at the endless respawning] ....
... as i said before, Thief DS [shalebridge] and AvP II [marine mission] are "scarier" ... intense to the point of just
having to exit the game to take a 'break' from jangled nerves.
