Another vote for Ninja Gaiden. Any of them. Although the original (for the NES) and Black would get my votes as the toughest of all time.

Ha, I actually snapped an Original NES controller in half playing this fvcking game as a kid. I could have easily gone on a serial killing rampage after a few hours of this one.

Castlevania 1 for NES was a MFer because you could not save your game. If you shut the console off, the game started at the beginning again. I think my buddy and I started at 4PM and played it all the way through in 12 hours. That last Dracula is a BIATCH unless you get the boomerang and the III and the pile of big Hearts

Superb games though.
