Probably google. If you allow cookies, and sign in to their services, they know who you are. Cookie management should help a lot. Contrary to the above, I wouldn't recommend ghostery. It's proprietary software, made by an ad company. I use ublock(origin) with aggressive lists, SelfDestructingCookies, block third party cookies, and use NoScript. For a little redundancy I have PrivacyBadger, but it doesn't have much to do after using my other stuff. Oh, and I don't use google aside from legacy email(handled with thunderbird).

edit:
It goes without saying, facebook is spyware. They aspire to be as intrusive as google, but aren't quite there yet since their breadth of services isn't as wide as google's.

I don't use much of Google but do have a Google account (mostly for Youtube and adsense, which I actually want to stop using at some point as I don't like to facilitate their spying) and if I'm not logged in at some point it ends up logging me in automatically, so I guess it could be that. I do use FB too, I really should not, but that's pretty much my main communication tool as everyone is on there. It would be interesting if someone wrote a program that scrapes it as you so you can still view what people post, chat with them etc, but without actually going to their site or logging in. Wonder how hard that would be to do... it would probably be illegal, but that's why you make it a desktop app and not a website. FB is also horribly coded, so an app that acts as a better front end to it would be great.

For general privacy does Privacy Badger on it's own help a lot? I was reading up on it actually. I might get rid of no script which is a royal pain, and install privacy badger instead.

Imo, PrivacyBadger is best for someone like your mother. It's a set/forget product, and causes few issues. It's proactive and behavior based, so it doesn't require list maintenance. It doesn't have the power of NoScript though. Privacy and security are worth a little bit of work. If they aren't worth work, I don't know what is.

If you really want to be lazy, I'd use PrivacyBadger, and NoScript in allow all mode. That'll still help protect against some xss attacks, and you can slowly build a blacklist as you research various scripts sites use. Also, use some privacy oriented adblock lists. That'll stop a lot of the web bugs that track you from site to site. Dunno if they affect routine site use, but they don't seem to.
