she's stretching and adjusting her pants... there is no phone. and it happens in 3 seconds. Taking her eyes off the stroller and adjusting her pants is not a parenting fail, I'm sure she didn't expect the stroller to just roll straight off the platform that fast.

Not really... Your first priority is to secure the stroller, You either chock the wheel with your foot, or use the provided wheel lock. I've never had a stroller move more than a couple of inches when under my supervision.
