Being forced to kill the doctor was extremely immersion breaking because you realize it's a game and then explore the potential options you think the game may have. That part should definitely have been a cut-scene.
Depends on how you look at it. Several people here have stated and I myself on my first play through didn't even hesitate to put a quick shotgun shell through his chest.So the idea that you have to take him out doesn't see to be that far out from what some people would do instinctively.
Infact not only did I shoot the doctor instantaneously but I didn't give either of the nurses a second thought as I ran up to the table and grabbed Ellie. It wasn't till the second playthrough where I watched them more carefully and noticed them cowering in the corner (shot them, but it was more of the not wanting to waste ammo mindset). Seems pretty right for Joel shoot the person in between you and Ellie without second thought and grab and run. When you really think about it there wasn't much of an alternative. With soldiers bearing down on him its not like he could sit there and talk the doctor down.
