You would think it would be obvious to the rider if the quick-release lever could come in contact with something.
My Trek doesn't have disc brakes, but I always line up the quick release lever with the metal of the fork. I'm guessing this issue is when it's lined up away from the fork, and inclines inward somehow (which would normally be tightening, so it's strange).
I think people put it on too loose and it bounces into the disc brake. I do what you do, adjust the QR until its very tough to tighten, and line it up with the fork. I kind of just mindlessly do it that way, after this I'll pay more attention. I don't ride a Trek, but I could see this happening on disc brakes in general.
