If a Video Card is shipping with a free title, will it explicitly state that in say a Newegg listing or is there another source to figure out what cards ship with what titles?
Yes, absolutely. It should either be a part of the SKU (such as AMD' BF4 tie-ins), or very clear verbiage on the e-tailer's site. It may even show up as a free "gift" in your shopping cart at checkout.
Be warned, lots of card won't actually come with the codes. You have to contact seller to get codes. Amazon.com has been this way in the past, my 780 TI I had to contact them for codes. Lots of people in comments for the card never got codes because promotion ended or ran out of them.
Battlefield 4 is included in the box for the 290x. All of the nVidia games are actually a card that they provide to the retailers that they have to ship out to the customer with the video card (hence they get lost/stolen/forgotten a lot).
